1. Purpose of This Consent

This document provides you with important information about telehealth services offered through RETA MD, operated by Reta MD Holdings LLC. By using our telehealth platform, you acknowledge that you have read, understood, and agree to the terms described below. Telehealth involves the use of electronic communications to deliver healthcare services at a distance. This includes evaluation, diagnosis, consultation, treatment, and the prescribing of medications by licensed healthcare providers.

2. Nature of Telehealth Services

RETA MD provides telehealth consultations for chronic weight management using tirzepatide, a prescription GIP/GLP-1 dual receptor agonist medication. Our services include: • Asynchronous and synchronous medical evaluations • Review of medical history, current medications, and health questionnaires • Clinical assessment to determine eligibility for treatment • Prescription of compounded tirzepatide when medically appropriate • Ongoing medical monitoring and follow-up care • Dosage adjustments based on clinical response and tolerability

4. Risks & Limitations of Telehealth

If you experience a medical emergency, call 911 immediately. • Not identical to in-person care: Telehealth consultations may not be equivalent to in-person visits. Some conditions may require an in-person examination. • Security risks: Despite reasonable safeguards, electronic communications carry some risk of unauthorized access to your health information. • Prescribing limitations: Your provider may determine that telehealth is not appropriate for your condition and may recommend an in-person visit instead.

5. Technology Requirements

To participate in telehealth services through RETA MD, you will need: • A device with internet access (computer, tablet, or smartphone) • A valid email address for communication and account management • Ability to receive text messages for appointment reminders and updates • For video consultations: a device with camera and microphone capabilities, and a stable internet connection All telehealth communications are conducted through HIPAA-compliant platforms to protect your health information.

6. Patient Rights & Responsibilities

You have the right to: • Receive the same quality of care as an in-person visit • Access your medical records at any time • Refuse or withdraw consent for telehealth services at any time without affecting your right to future care or treatment • Request an in-person visit instead of a telehealth consultation • Be informed of all parties present during a telehealth session • Privacy and confidentiality of your health information Your responsibilities include: • Providing accurate and complete medical history information • Informing your provider of all current medications, allergies, and health conditions • Following your treatment plan as prescribed • Reporting any side effects or adverse reactions promptly • Notifying RETA MD of any changes in your health status

7. Emergency Protocols

RETA MD telehealth services are NOT for medical emergencies. • If you are experiencing a medical emergency, call 911 immediately. • If you experience severe side effects from your medication, go to the nearest emergency room. • For urgent but non-emergency medical concerns, contact your RETA MD care team at hello@retamd.com during business hours. • After hours, contact your local urgent care facility or emergency services. Your healthcare provider will provide guidance on recognizing emergency symptoms related to your prescribed medication.

8. Privacy & Confidentiality

All telehealth consultations are conducted in accordance with HIPAA (Health Insurance Portability and Accountability Act) regulations. We use encrypted, HIPAA-compliant communication platforms to protect your personal health information. 9. Consent Acknowledgment

By using RETA MD’s telehealth services, you consent to the following: • You have read and understand this Telehealth Informed Consent document. • You understand the risks, benefits, and limitations of telehealth as described above. • You understand that telehealth is not a substitute for emergency medical care. • You consent to receive healthcare services via telehealth from RETA MD’s licensed healthcare providers. • You understand that your healthcare provider may determine that telehealth is not appropriate for your situation and may refer you to in-person care. • You understand that you may withdraw your consent at any time.
